Aberdeen Place Hospice is looking for an outstanding marketer to help establish and build our hospice program in and around the Atlanta metro area. This is a ground floor opportunity for a highly motivated, successful hospice or home health/personal home care individual. The Community Relations Manager is responsible for generating hospice referrals in a given territory by creating and building relationships with key referral sources: physicians, nurses, discharge planners, social workers, insurance/financial/legal professionals, hospitals, assisted living communities, personal care homes, nursing homes, home health/home care agencies and other community-based referral sources. Responsible for marketing and maintaining positive relationships with customers/referral sources and responding to customer requests/concerns. Serves as the customer service representative for all hospice agency referral sources including sales calls, hospice education, and community in-services regarding hospice. Meet monthly goals. Attend sales, operational and staff meetings.

Aberdeen Place Hospice, a locally owned and growing area hospice, is seeking a highly motivated, compassionate, and energetic Registered Nurse with at least 3 years of hospice/home health experience to join our team of dedicated professionals for a full-time position. The Hospice RN establishes a professional and caring relationship with our patients and families, support staff, physicians, discharge planners, social workers, facilities and other health care professionals by providing hands-on care, on-going clinical management and education regarding hospice care.
Qualifications for this position are:
Registered Nurse with a current Georgia RN license
Current Georgia driver’s License and automobile insurance
CHPN certification a plus
Excellent people, verbal and written communication skills
Good computer skills
Ability to think on your feet
As a member of the interdisciplinary team, the Nursing Assistant will provide personal care services to the terminally ill patients and perform related tasks in accordance with the plan of care as assigned by the RN Case Manager.
Qualifications:
Must meet Federal and state Hospice Aide training and competency requirements.
One year working as a Certified Nursing Assistant (CNA).
High school diploma or GED.
Completion of a CNA Certification Training Program with certification by the Georgia Board of Nursing.
Current registration as a CNA in the State of Georgia.
Emotional maturity, good physical health and dependable transportation
Ability to listen, read, and write English and follows directions.
Ability to perform tasks independently, under directions/supervision of the RN Case Manager.
Ability to work a flexible schedule.
Must possess a valid Georgia driver’s license and proof of automobile insurance.
Responsibilities:
Responsible to carry out the duties, assigned by the RN Case Manager, for a hospice patient caseload. Carry out duties assigned in order to increase patient’s level of comfort, to maintain personal hygiene and a safe environment. All care provided under the direction of the RN Case Manager and in accordance with the plan of care.
